The period after a cardiac event or procedure involves new medications, changed activity advice and a hospital discharge summary that has to be translated into ongoing care. 03What to bring
Bringing Your Hospital Documentation to the First Appointment
This appointment depends more heavily on paperwork than most. Your cardiologist needs the discharge summary, the procedure or catheter report, any imaging performed in hospital and the discharge medication list, because these set out precisely what was done and what was planned to follow. Bringing them, or having them sent ahead, prevents the first consultation being spent reconstructing the admission. Your cardiologist will review that documentation, examine you, ask about symptoms since discharge and check what you're currently taking against what was prescribed. They'll then set out the follow-up schedule, which investigations are due and when, and what remains with your GP between specialist appointments. Correspondence goes to your GP and, where relevant, back to the treating hospital team.
