Managing Conditions That Don't Resolve

Diabetes, heart disease, kidney disease and long-term respiratory conditions aren't resolved in a single consultation. They're reviewed and adjusted over years. At Nuevo Medical, chronic disease management on the Gold Coast will run through our GPs, with pathology collection, imaging and allied health in the same building so the review cycle doesn't break down between appointments. Much of the work is unglamorous: confirming the bloods were actually done, checking whether medications are still appropriate, noticing what has changed. That's easier when the collection room is a short walk away. Patients managing a long-term condition are often also working on cardiometabolic risk. Call 1300 068 386 to book an appointment.

Conditions Our GPs Will Manage

Our GPs will look after a broad range of long-term conditions, drawing on specialist input where it's needed.

    Type 2 diabetes

    Regular review of glucose control, kidney function and cardiovascular risk factors. Onsite pathology means the bloods can be collected on the day they're ordered.

    Cardiovascular disease

    Ongoing management of blood pressure, lipids and medications, with cardiology available in the same centre. Review frequency is set according to how stable things are.

    Chronic kidney disease

    Monitoring of renal function and electrolytes, alongside review of any medications that affect the kidneys. Testing intervals are determined by the treating GP.

    Respiratory conditions

    Review of asthma and chronic obstructive pulmonary disease, including inhaler technique and written action plans. Chest imaging is available onsite where it's indicated.

What a Chronic Disease Review Appointment Covers

A review appointment runs longer than a standard consultation because it covers more ground. Your GP will work through recent pathology and imaging, check your current medication list against what you're actually taking, and ask about symptoms, side effects and anything that has changed since the last visit. They'll also review correspondence from any specialists or allied health practitioners involved. It helps to bring a current medication list, including anything bought over the counter, and any readings you've been recording at home such as blood pressure or glucose. After the review, your GP will discuss which investigations are due, whether any medication needs adjusting and when the next appointment should be. Where specialist input is appropriate, the referral is arranged during that consultation.

That depends entirely on the condition and how stable it is, and it's a decision made by the treating GP. Intervals are usually set at the end of each review rather than left open.

Many long-term conditions and the medications used to manage them are tracked through pathology rather than symptoms, because changes often show up in results before they're noticeable. Testing intervals are set by the GP based on the specific condition.
